When it comes to designing response format for questionnaire, it could be structure from open-ended questions, multiple choice or dichotomous.

There is no one single format that works best. All response formats has its advantages and disadvantages.
Relearn
Open Ended Questions: It allows attitudes to be expressed but it also could lead to high potential of interviewer bias.
Multiple Choice Questions: Reduce interviewer bias but it could also be viewed as bias due to the alternatives response provided.
Dichotomous Questions: It is simple and provides clear answer for example yes or no. However it forces a choice between only two alternatives.
